Unscheduled treatment
This is a short patient letter for treatment that was diagnosed but never booked. Written on office letterhead, it reminds the patient that the doctor diagnosed treatment at the last visit, invites questions about the treatment or insurance coverage, and asks them to call to schedule. Blanks cover the patient’s name, the doctor’s name, and the signing staff member’s name and email.
Send it as part of your unscheduled treatment follow-up, after phone calls have not reached the patient or the patient said they would call back and did not. Front desk staff or the treatment coordinator can mail or email it from a report of open treatment plans, then note in the chart that the letter went out.
