Vacation request form
A simple request form for vacation or personal days, printed on office letterhead. The employee fills in the date, name, office title, the vacation or personal time already earned, the dates requested, and the day they plan to return, then signs. The bottom section is for the dentist or office manager, who records the total days or hours requested, marks the request approved or not approved, explains any denial, and signs.
Use it whenever a team member wants time off, so every request is in writing and lands in the employee file. It lets the office manager check coverage on the schedule before approving and gives a clear record if questions come up later about earned time or overlapping requests.
