An email template sent after a consultation once the patient’s insurance has confirmed coverage. It tells the patient the estimated out-of-pocket cost, asks for a deposit of half that amount to schedule, and says the balance is due on the day of the appointment. It offers two appointment options and mentions an electronic payment link or paying by card over the phone.

Treatment coordinators can send it once the estimate is ready, so the patient has the numbers in writing. Fill in the amounts and times, attach your payment link, and follow up by phone if there is no reply within a day or two. Save a copy of the sent email in the patient’s record.
