Dental fee increase letter

A letter to an insurance company’s provider relations representative asking to renegotiate fees. It thanks the carrier, notes roughly how many of its members the office has treated during the contract, and proposes a meeting about the fee schedule for the next term. Blanks cover the insurance provider, doctor, and patient count.

Send it before a PPO contract renews. The office manager or owner fills in real numbers from the practice reports, edits the reasons for the request to fit the office, and follows up by phone if the carrier does not respond. Keep a copy with the contract so the date of the request is on record.
