Sample Form: Objection Crusher Form

A worksheet for the moment a patient hesitates after a treatment plan discussion. It records the patient’s name, the date, and the proposed treatment, then lets staff check the initial objection, such as needing to think about it, cost, talking to a spouse, fear, or doubting the urgency. It lists follow-up questions to ask, with space for notes from the conversation.

The form suggests keeping a printed copy at the treatment coordinator desk or in the front desk binder and using it during or right after the discussion. Treatment coordinators and front desk staff can file the completed sheets and review them later to see which objections come up most and how the follow-up calls went.
