periodontal_charting

A blank periodontal chart for recording probe readings. It has rows for three exam dates on both the facial and lingual sides, upper teeth numbered 1 to 16 and lower teeth 17 to 32, with three readings per tooth. There are lines for upper and lower mobility, the patient’s name, clinician initials, and a section for dates and significant findings.

Hygienists and dentists can use it when the practice charts on paper, during a software outage, or as a copy to send with insurance claims that require charting. Keeping three dates on one sheet makes it easier to compare readings over time and show a payer how the patient’s condition has changed.
