Oral cancer visual screening detection consent form

A patient handout and consent form for oral cancer screening with the OralID system, printed on office letterhead. It lists warning signs, such as sores that do not heal within two weeks, lumps, white or red patches, and trouble chewing or swallowing. It explains that OralID uses natural tissue fluorescence as an aid in the early detection of mucosal abnormalities and recommends the screening for patients who use tobacco.

Give it to patients before the screening is offered, typically at a hygiene or exam visit. Hygienists and front desk staff can use it to explain the screening, and the signed form shows whether the patient accepted it. Check that the statistics in the handout still match current sources before printing.
