Narrative for Endodontic Therapy

A narrative template for root canal claims, addressed to the dental claims consultant. It lists codes D3310, D3320, and D3330 for anterior, bicuspid, and molar endodontic therapy, then gives indications such as pain, pulp exposure or decay, and infection or abscess. A diagnosis section offers options including irreversible pulpitis, nonvital tooth, and necrotic pulp, plus a line confirming the tooth is stable with a favorable prognosis.

Use it when a carrier asks for documentation on endodontic treatment. The insurance coordinator marks only what the chart supports, adds the patient details, and sends it with the supporting records. Have the treating doctor review the diagnosis section first.
