Informed consent crown and bridge

An informed consent form for crowns and fixed bridgework. It states that the treatment carries risks and that no result or lifespan is guaranteed, then lists numbered risks such as the need to reduce tooth structure, sensitivity after preparation that may be short or long term, and the chance that a crowned tooth may later need root canal treatment.

Have the patient sign it at the preparation appointment or when the treatment plan is accepted. The assistant reviews the risks while the doctor is available for questions, and the signed form goes into the patient’s record. Keeping it on file helps if the patient later asks why a crowned tooth needed more work.
