Electronic communication form
An electronic communication consent form for patients. The patient initials to agree or not agree to receive electronic messages and acknowledges the risk that unencrypted email can be intercepted. They choose text or email as their preferred method and pick what they want to receive, such as appointment reminders, insurance and billing information, and satisfaction surveys, and agree to keep their contact details current.
Add it to the new patient packet and offer it to existing patients before turning on text or email reminders. The front desk records the choices in the practice management system so messages only go to patients who agreed.
