Chemoclearance
A short letter for a patient undergoing chemotherapy. After an oral exam, the dentist states that the patient needs no further dental treatment at this time and recommends routine exams and cleanings at an interval the dentist fills in. Fields cover the date, patient name, and date of birth, with a signature line for the doctor.
Use it when an oncologist or medical team asks for dental clearance before or during chemotherapy. The front desk prepares it after the exam, the doctor signs, and a copy goes to the requesting office and into the chart. Schedule the recommended recall visits before the patient leaves.
